Mobile Treatment Team (ACT)

Our Mobile Treatment Team delivers intensive, community-based care using the evidence-based Assertive Community Treatment (ACT) model. The team includes clinical, medical, and support specialists who provide 24/7 crisis response, small caseloads, and personalized services in the community. ACT Fidelity Status confirms we meet the highest standards of care, helping individuals stay housed, avoid hospitalization, and improve quality of life.

ACT services are available to adults over 18 who are part of the priority population for mental health care. This includes individuals with a serious mental health diagnosis who have struggled to engage successfully in traditional outpatient treatment. Eligibility may also include frequent use of crisis or psychiatric services, legal involvement related to mental illness, or experiences of chronic homelessness.

Project for Assistance in Transition from Homelessness (PATH)

Our PATH program provides outreach and case management to connect individuals experiencing homelessness with mental health treatment and recovery supports. We partner with shelters, state agencies, behavioral health providers, and community organizations to ensure people receive the care and resources they need.

PATH services are available to individuals with a mental health diagnosis who are experiencing or at risk of homelessness. This may include those facing eviction without another residence, being discharged from an institution without housing, lacking a permanent nighttime residence, fleeing domestic violence, or residing in a shelter or transitional housing.

We accept all major insurance plans including Medical Assistance, Medicare, and private insurance. If you are uninsured or underinsured, we offer sliding-scale and self-pay options. We are committed to making care available to everyone, regardless of financial circumstances.

That is okay. We understand that not everyone has coverage, and we will work with you to make services affordable. No one is turned away due to inability to pay.

We often recommend combining therapy and medication, but we understand that every person has different needs. Everyone starts with a clinical intake, and if medication is appropriate, we will refer you to a psychiatrist or nurse practitioner for evaluation and ongoing care.

Substance use disorders can affect every part of life, from health to relationships and work. We provide compassionate, person-centered care to support lasting recovery. Care begins with a professional assessment to understand each person’s needs, followed by individual counseling, weekly therapy groups, peer support, and coordinated services. Many people face both substance use and mental health challenges, known as co-occurring disorders, and our integrated approach treats both together for the best outcomes.

Our team of therapists, addiction counselors, peer specialists, and medical providers work together to guide recovery. Services include comprehensive assessments, individual and group counseling, relapse prevention and recovery skills, treatment for co-occurring conditions, psychiatric care and medication management, peer support, and community resources. We welcome individuals of all backgrounds, accept most insurance, and offer sliding-scale options. Recovery is possible, and you do not have to face it alone. We are here every step of the way.

Corsica River Mental Health Services (CRMHS) provides Level 1 Outpatient Substance Use Disorder (SUD) Treatment for individuals ages 12 and older at all three locations. To begin services, clients complete a referral form and questionnaire, followed by a substance use assessment to determine if Level 1 treatment is appropriate.

Psychiatric Rehabilitation Program (PRP)

Our Psychiatric Rehabilitation Program (PRP) helps children, young adults, and adults build life skills, emotional wellness, and independence. Children and youth join after-school mentoring and group activities that strengthen coping, communication, and social skills. Young adults ages 16 to 25 receive support with education, employment, and healthy decision-making, while adults focus on recovery skills, emotional stability, and building support systems. Along with these core areas, the program offers other individualized supports at our Recovery Club locations or in the community.

To join the Psychiatric Rehabilitation Program, you need a referral from your current therapist, psychiatrist, or another licensed mental health provider. Referrals can come from Corsica River or any other qualified provider. The program is for people who have a mental health condition and would benefit from extra support in daily life.

Residential Rehabilitation Program (RRP)

Our Residential Rehabilitation Program offers supportive housing for adults 18 and older living with serious mental health conditions. The program focuses on building the skills needed for greater independence in the community. Residents receive help with daily living tasks such as cooking, cleaning, laundry, personal care, and budgeting, along with support in managing medications and healthcare appointments. In this structured setting, participants work toward personal growth, stability, and long-term recovery.

Residential Rehabilitation Program services are available to adults 18 and older who have a primary behavioral health diagnosis such as schizophrenia spectrum and/or depressive and bipolar disorders.

All applications and questions about vacancies must go through the Core Service Agency, Mid-Shore Behavioral Health. Crossroads cannot accept referrals directly, and submitting an application does not guarantee placement.

Evidence-Based Supported Employment Program

Our Supported Employment Program helps people with behavioral health needs find and keep meaningful jobs, based on the belief that work is an important part of recovery. Participants receive personalized support with job searches, interview preparation, on-the-job coaching, benefits counseling, and budgeting, along with vocational assessments and coordination with behavioral health providers, employers, and family members. We also partner with agencies such as the Division of Rehabilitation Services and the Department of Labor to connect individuals with resources that support long-term success in the workplace.

The Supported Employment Program is open to individuals 16 and older who are currently receiving mental health treatment and would benefit from help with finding or keeping a job. Participants should have an interest in working and a willingness to take part in the job search and employment process with the support of our team.

Respite Care

Our Respite Care Program provides short-term, out-of-home care for children who are receiving behavioral health services, giving parents and caregivers planned relief while ensuring children remain in a safe, supportive setting. Care is provided in licensed homes across Maryland’s mid-shore region by trained providers who understand the needs of children with behavioral health challenges. Each child has a personalized Respite Care Plan to guide their stay, supporting consistency and continuity of care. This program is planned in advance and is not an emergency service.

Respite Care is for children under 18 who are in behavioral health services and live with a parent or guardian. It is free for families with Medical Assistance, and a behavioral health provider must approve the referral.

Health Home

Health Home is a community-based program that supports overall wellness by bridging the gap between physical and behavioral healthcare. The program helps individuals manage chronic conditions, avoid preventable hospital visits, and stay connected to the services they need for long-term health and stability. Participants receive personalized care coordination that may include care management, health education, support during transitions between care settings, and connections to community resources. Our team works closely with each person’s primary care provider, specialists, and behavioral health professionals to create a unified plan of care.

To participate in Health Home, individuals must be enrolled in either our Psychiatric Rehabilitation Program (PRP) or Residential Rehabilitation Program (RRP). There is no separate referral process. Anyone in these programs is automatically considered for Health Home as part of their care plan.

Art Program

Our Art Program offers a creative outlet for self-expression, healing, and connection for individuals receiving behavioral health services. Through painting, drawing, and other forms of visual art, participants build confidence, develop coping skills, and engage in meaningful social interaction in a supportive and therapeutic environment.

Each year, clients have the opportunity to share their work alongside local artists during our True Colors Mental Health Awareness Art Show, held every October in recognition of Mental Health Awareness Week.

The Art Program is available to participants in our PRP or RRP. No separate referral is needed, as eligible individuals are included through their care plan.
